Transfer
$135 certification fee
- An equivalent certification from a recognized U.S. certifying agency, active or expired within the last 12 months
- Copy of your diploma or certificate of completion
- The board reviews the equivalency of your certification

Experience
$90 certification fee
- 1 year of verifiable full-time work as a Home Health Aide
- Experience gained within the last 5 years
- Supporting documents requested by the board
